How We Improve Health

At UCAN, we recognize the profound impact that non-medical factors, often referred to as social determinants of health, have on the overall well-being of individuals and communities nationwide.

Every program operated by UCAN provides a service that improves health.

Non-medical factors are what shapes people’s lives from birth onward, and they wield far-reaching influence over health outcomes, surpassing the significance of clinical care. The U.S. Office of Disease Prevention and Health Promotion classifies these factors into five key areas:

  • Economic Stability
  • Education Access and Quality
  • Healthcare Access and Quality
  • Neighborhood and Built Environment
  • Social and Community Context

We're committed to improving health outcomes.

Our commitment to improving health outcomes extends to various facets of these non-medical factors, including:

Addressing Economic Stability

Promoting Education Access and Quality

Strengthening Social and Community Context

Enhancing Healthcare Access and Quality

Factors Related to Neighborhood and Built Environment

We not only move people out of poverty, we prevent health issues that otherwise burden our community's healthcare system.

Adding Services Addressing Health

We are further sharpening our focus on improving local health through two initiatives. We are now a provider of Medicaid 1115 Waiver Health Related Social Needs Services (HRSN). By participating in Oregon’s waiver, we are able to offer more of the services we are known for, like rental assistance, while adding new services such as installation of wheelchair ramps and more. We are also currently offering Healthy Homes services through funding from the Oregon Health Authority. This funding allows us to improve indoor air quality by offering air purifiers and filters. These efforts can directly reduce the incidence of asthma and other chronic conditions.
