Family Connects

All babies deserve the healthiest possible start. Family Connects Oregon is a free, evidence-based, nurse home visiting program for all families with newborns, including foster and adoptive families. Nurses provide support and resources so that newborns and their parents can flourish.

Services

Our Family Connects nurses can:

  • Check your baby’s weight,
  • Check your baby’s health,
  • Check mom’s health,
  • Help mom manage anxiety and post-partum depression,
  • Offer tips on how to feed your new baby,
  • Offer tips on how to help your baby sleep,
  • Provide referrals to other services to support your family and your new baby, including:
    • child care,
    • family planning, and
    • planning whether to go back to work.

To Qualify

All birth parents, foster parents and adoptive parents of newborns living in Douglas County qualify for Family Connects. It makes no difference where the baby is born. It also makes no difference how many children a family already has-we offer Family Connects to families of any size household who are caring for a new baby. There is no income requirement for this program. The program differs from our Nurse Home Visiting program, in that you do not need a special referral to take part in the program.

Our Process

1. You will be given a form to request Family Connects at the hospital which you can return to your nurse there. You can also ask your nurse at the hospital to refer you to our program.

2. If you don’t sign up for Family Connects at that time, you can still obtain the service by having your pediatrician refer you, or by completing the UCAN form found here.

3. We only need your health insurance info to start offering the service.

4. We will usually first see you when your baby is 3-4 weeks old. If an urgent need comes up, we can see you and your baby sooner.

5. We offer up to 3 visits, each lasting 1-2 hours, and can visit up until your baby is 3 months old.
